Provide timely notification to the Health Authority of its intent to assume or abandon its jurisdiction but in no case provide less than one years’ notice of such action or at a time mutually acceptable to both parties. C. In the event of abandonment of jurisdiction, agree to cooperate with the Health Authority in the transfer of responsibility including timely transfer of all records maintained by the municipality.
