Bryson <br />GENERAL CONTRACTOR: SEPTIC CONTRACTOR: <br />SITE EVALUATOR: s- P Testing <br />The City of Orono has <br />Approved <br />(approved -disapproved) <br />REPORT DATE: :%pri 1 16, 1995 <br />your on-site system design as of June 29, 1995 <br />(date) <br />Willi die following comments: A legal agreement must be in force regarding the <br />use and maintenance of the septic system prior the issuance of a septic <br />permit. Also, an easement over the septic system area must be provided <br />for the lone term use by each residence. <br />THIS IS NOT A PERMIT. This is a design approval form which must accompany the site plan. A permit must <br />be issued to a licensed septic contractor prior to installation. A list of currently licensed septic contractors is <br />enclosed. <br />NOTICE TO INSTALLERS: Any changes to the approved plans and specs must have prior approval of the <br />Inspector (473-7357). Call for inspections 24 hours in advance. <br />ALL DRAINFIELD AREASMUST BE FENCED OFF prior to building site excavation and fencing must remain <br />in place until final site grading. Approval to pour footings will not be granted until the Inspections Department <br />has verified that primary and alternate sites are adequately protected. <br />NO VEHICULAR TRAFFIC OF ANY KIND (cars, trucks, earth moving equipment, etc.) is allowed within 20' <br />of tested drainfield sites either before or after system construction.
