(The sample resolution will have to be <br />modified appropriately by each member). <br />As stated in the sample Resolution, LOGIS has, since 1988, been administering a group health and medical <br />insura:ice plan (Plan) for those members who have chosen to participate. LOGIS' legal counsel has advised <br />the Board of Directors that the Agreement should be modified as proposed by the Amendment to clarify <br />LOGIS’ authority to conduct and administer the Plan. The amendment also provides that other types of <br />group benefits may be included in the Plan. The resolution also ratifies the participation in the Plan by <br />members of the Plan. <br />It is necessary for all members, including those governmental units that have not participated in the Plan, to <br />approve the Amendment. The Joint Powers Agreement can be amended only by a unanimous action of the <br />members. Once you have adopted the Resolution, please mail a certified copy with the Amendment attached <br />as Exhibit A to Yvonne Johnson, Stanton Group, 3405 Annapolis Lane N, Plymouth, MN 55447. We will <br />file the copy on your behalf with the City Manager of Brooklyn Center as provided in the original <br />Agreement. <br />In addition, a new LOGIS Group Health and Life Insurance Policies and Procedures document has been <br />included for your information.
