EVENT PERMIT Dite Issued: Date of Event: September 12,200S Saturday, September 17,200S Type of Event: Time: Location of Event: Fundraiser - Steak Fry with Live Music (Temporary On-Sale Liquor License Issued to Oronu Lions) 5:00 p.m. - Miutugbt 340 Willow Drive North (I Name of Person(s) And/or Organisation Sponsoring this Event: Long Lake Fire Department Relief Association 340 Willow Drive North Orono, MN SS3S6 Phone: 952-473-9701 The following documents are required and on file at the City Administrative Offices: • Approval from Orono Police Department The following conditions have been placed on this event: • By acceptance of this special event permit, the permit holder, on behalf of any and all organizations and private persons, is i- granted authority to operate under the special event permit, and agrees to indemnify and hold harmless the City of Orono from all claims arising from said event. The permit holder, all organizations and private persons exercising authority under this permit, do waive and release all claims against the City of Orono, its officers or employees for any damage to person or property arising from the exercise of privileges granted by this permit and agrees to hold harmless the Cit>- of Orono, its employees and officers from any such claiia The Orono Police Department and its officers are empowered to revoke this special event permit at any time for any safety concerns diat are not immediately resolved by the permit holder or a representative of the permit holder. This revocation shall cause the immediate cancellation of the event originally authorized by this permit. Police may issue citations or arrest for any violations of law as appropriate. Live band is to be located inside the fire station. Music is to end no later than 2359 hours on Sanirday, September 17,2005. Parking is to be in the City of Orono lot to the east of fire station. Long Lake Fire Department is to mow the grass and weeds to open up the parking area. • No parking is to be allowed on V/illow Drive. Fire Department is to post signs fiom south of the railroad bridge to Glendale on both sides of Willow Drive. • The scuth road to the fire station is to be posted no parking to allow free access to and from the parking area and the station. • Any event-related debris is to be cleaned up by the following day. o Two Orono Police Officers ar to be hired to police the event. • A Certificate of Insurance must be provided naming the City of Orono. i»s agents and employees as additional insureds and providing public liability insurance in the amount of at least S300.000 for injury of o.ne person. 5500,000 for injury of rivo or more, and 550,000 for property damage. • Any signs that are posted must comply with all City of Orono ordiruinces. • Orono Police are empowered to revoke this permit at any time and cancel the event for any alcohol violations.% .Approved at City Council meeting on: September 12.2005 (City Seal) Ronald J.
