CITY OF ORONO SPECIAL EVENT PERMIT Date luucd: Date of Event: June 13,200S Tuesday, June 14,200S Type of Event: Time: Location of Event: Art of Living Gala and Auction (Food, beverages, raffle and music) 6:30 p.m. to 10:00 p.m. 2240 North Shore Dnve, Minnetonka Center for the Arts r Name of Personfs) And/or Organization Sponsoring this Event: John Greving Sister Kenny Foundation 800 E. 28* St., #16509 Minneapolis, MN SS407 Phone: 612-863-4618 The following documents are required and on file at the City Administrative Offices: • Approval from Orono Police Department • Approval from Orono Building and Zoning Department The following conditions have been placed on this event: • By acceptance of this special event permit, the permit holder, on behalf of any and all organizations and private persons granted authority to operate under the special event permit, agrees to indemnify and hold hatmless the City of Orono from all claims arising from said event. The permit holder, all organizations and private persons exercising authority under this permit, do waive and release all claims against the Cit>' of Orono, its officers or errqrloyees for any damage to person or property arising from the exercise of privileges granted by this permit and agrees to hold harmless the City of Orono. its employees and officers from any such claim. • The Orono Police Department and its officers are empowered to revoke this special event penrut at any time for any safety concerns that are not immediately resolved by the permit holder or a representative of the permit holder. This revocati.m shall cause the immediate cancellation of the event originally authonzed bv this permit. • Event parking to be coordinated with a private valet service and the Orono Police Department. All vehicles must be legally parked, ..legally parked vehicles may be towed. • Live music to be allowed from 6:30 p.m. - 10:00 p.m. • Temporary One Day On Sale Liquor License is required. • Gambling Permit is required. • The contact name and number dunng the event must be confimied wnh the Orono Police Department (Charlotte Flowers-Huebner at 651-442-5595). The contact phone number must be a cell phone for an event sponsor who will be at the event and en^wered to resolve any conflicts on behalf of the sponsor • Notification to be provided to the neighborhood regarding the event activities, hours, and a contact name and phone number for any questions or concerns regarding this event. • Permit holder is responsible for inspecting the area after the event and repairing any danuge caused by- parking. removing any event-related signs, and cleanup of any event-generated debris Approved at City Council meeting on: June 13.2005 (City Seal) Ronald J.
