Box 66
Orono, MN 5535 Crystal Bay, MN 55323-0066
September 12, 1996
I
Sullivans Utility Services, Inc.
3660 Highway 101 S.
Wayzata, MN 55391
Dear Mr. Sullivan:
I am sending you this letter to document my observati ns during three inspections of the septic
system at 4440 Bayside Rd. earlier this year. My first ins ection was performed in early May with
the homeowner at which time I observed that the first se tic tank manhole had been dug up and a
new plastic riser was in place rather than the concrete riser than had been there in the past. The other
two septic tanks appeared to be unaltered from when the s stem was originally installed. The pump
tank riser had stains running down the riser, indicating hat water was entering the tank from the
riser. The drainfield was discharging effluent from the las trench. At this time the system was non-
compliant with current City and State codes due to the s rface discharge.
My second inspection was performed June 28th. At this ime I observed that the second and third
tanks now had new plastic risers. Some additional soil ha been placed over the drainfield to drain
surface water downslope (and away from the system). T e drainfield was not dischaging effluent
to the surface and therefore was compliant with current ity and State codes.
My third inspection was performed August 26, 1996. T e inspection revealed that the septic tank
risers now show no evidence of leaking, but the drainfield trenches are overloaded. The effluent in
the inspection riser on the last trench has a liquid level fi e inches below the ground surface. The
drainfield is not discharging to the surface at this time, ut has a high potential for failure. The
system is classified as compliant at this time.
Please contact me at the City Offices if you have any qu stions.
Sincerely„
c1 _ -_l`"

,.. ,--
Stephen eckman
On-Site Systems Manager
Telephone (612) 473-7357 F X 473-0510
