STATE OF MINNESOTA <br />ss. <br />COUNTY OF HENNEPIN <br />AFFIDAVIT FOR COLLECTION <br />OF PERSONAL PROPERTY <br />Affiant, Tabitha Coffindaffer, being first duly sworn, deposes and states: <br />1 Affiant is a daughter of the decedent, Clarence Coffindaffer. Affiants address is <br />P.O. Box 267, Dellslow, WV 26531, <br />2. Clarence Coffindaffer was found dead at the age of 77 on August 23, 2019, and <br />at the time of his death resided in the County of Hennepin, State of Minnesota <br />with a Social Security number of 233-70-6578. <br />3. Affiant has possession of tangible personal property. <br />4. The value of the entire estate, wherever located, less liens and encumbrances, <br />does not exceed $75,000.00. <br />5. Thirty (30) days have elapsed since the death of the decedent. <br />6. No application or petition for the appointment of a personal representative is <br />pending or has been granted in any jurisdiction, <br />7. Affiant, the claiming successor, is entitled to payment or delivery of the following <br />described property, to wit: State of Minnesota tax refund, Met Life Auto Home <br />reimbursement check, miscellaneous refund checks, two vehicles and furniture <br />on consignment. <br />FURTHER AFFIANT SAYETH NOT. <br />Dated'.
