ORONO WOODS SENIOR HOUSING <br /> PHASE 1 (SITE GRADING) PERMIT CONDITIONS <br /> READ NOW!! <br /> Attachment to 2040 W. Wayzata Blvd. (Orono Senior Housing) Phase 1 grading permit. <br /> Terms and Conditions of Permit <br /> 1. This permit is subject to the terms and conditions of the PUD 2B Agreement,a copy of which <br /> is attached hereto. <br /> 2. Permit approval is based on the approved Site Plan(Sheet C2-1, latest revision 12-17-01) <br /> and the approved Grading/Drainage/Erosion Control Plan (Sheet C3-1, latest revision 12- <br /> 17-01) and is subject to ongoing review and site inspections by the City. Any changes to <br /> plans require City approval. <br /> 3. Orange silt fence shall be installed and maintained in place and in functional condition at <br /> all times, subject to Minnehaha Creek Watershed District requirements. <br /> 4. Tree removal shall occur only within the limits established by silt fencing as shown on <br /> the approved plans. <br /> 5. In the event that site activities result in spillage of fill materials on adjacent roadways, <br /> contractor shall be responsible for maintaining area roadways in a clean condition on a <br /> daily basis or as may be required by the City. <br /> 6. In the event that excess fill is to be hauled off-site, be advised that any location in Orono <br /> to be receiving such fill must first have obtained the appropriate permits from the City <br /> (more than 500 cubic yards to any single receiving site requires a 60-day conditional use <br /> permit review process). <br /> 7. This permit covers only the Phase 1 Improvements as listed in the attached Phase 1 Site <br /> Grading Cost Estimate. A separate permit will be required for the Phase 2 Improvements <br /> which include all work shown on the attached Phase 2 Utilities and Street Improvements Cost <br /> Estimate. <br /> 8. THIS PHASE 1 IMPROVEMENTS PERMIT WILL BE REVOKED IF THE PHASE 1 <br /> LETTER OF CREDIT IS NOT RECEIVED AT THIS OFFICE BY 4:30 P.M. ON <br /> WEDNESDAY, DECEMBER 26, 2001. IF THIS PERMIT IS REVOKED, THE CITY <br /> WILL ORDER ALL WORK AT THE SITE TO BE STOPPED BEGINNING AT 8:00 A.M. <br /> ON THURSDAY DECEMBER 27,UNTIL THE LETTER OF CREDIT IS RECEIVED AT <br /> WHICH TIME THE PERMIT WILL BE REINSTATED. <br /> 9. Commencement of the Phase 2 (Utilities and Street) Improvements may not commence <br /> until a Phase 2 Improvement Permit is issued, which we anticipate will be in the spring of <br /> 2002. Such permit will not be issued until the Phase 2 Letter of Credit is received. <br /> 10. This permit does not cover the following activities which require separate permits: <br /> - Foundation and building construction <br /> - Utility connections to building <br /> - Building systems (plumbing, mechanical, electrical, etc.) <br /> Sheet 1 of 4 <br />
