Employee agrees that Employee’s <br />termination of employment was voluntary and that the decision to quit was not the result of <br />a good reason caused by the City. Employee has been advised that an employee who <br />voluntarily quits is generally not eligible for unemployment insurance. <br /> <br /> 5. RELEASE. In consideration of the benefits provided by this Agreement, <br />Employee shall furnish the City a Release in the form attached hereto as Exhibit “A.” If the <br />Employee rescinds the Release, as provided for in the Release, this Agreement shall be null <br />and void. <br /> <br /> IN WITNESS WHEREOF, the City and the Employee have approved and <br />executed this Early Retirement Agreement on the day and year first above written. <br /> <br /> <br /> <br />Dated: , 2020.
