<br />WATER TANK MAINTENANCE CONTRACT <br />AMENDMENT SHORT ELLIOTT HENDRICKSON INC. 3 <br />Scope of Services <br />SEH D|B, Inc. shall, as an amendment to the original contract, provide services and specified work items and repairs <br />in accordance with the attached documents, including the following: <br /> Table A: Schedule of Services <br /> Table B: Specific Work Items and Repairs <br /> Contract Deliverables <br /> Performance Guarantees <br /> Table C: Schedule of Payment <br />WORK PLAN <br />The Work Plan under this extension will generally consist of three procedures (at different times during the contract), <br />which are defined as follows: <br /> Visual Survey: Annual visual inspections of the tank are <br />performed while the tank remains in service to determine the <br />general condition of the coatings and accessories. This work will <br />include a report summarizing the results of the inspection. <br /> General Maintenance: The wet interior and exterior surfaces will <br />be washed at scheduled intervals, as well as performing spot <br />coating repairs to needed areas of the tank. This may also include <br />minor modifications to the tank appurtenances and equipment. <br /> Engineering Evaluation: A detailed inspection of the tank is <br />performed to determine the specific structural and sanitary <br />conditions, as well as the condition of coatings systems relative to <br />accepted industry standards. This work will include the entrance of <br />personnel into the water storage areas to inspect all interior <br />surfaces. <br />The Engineering Evaluation will also include a report that describes existing conditions and presents <br />recommendations for future maintenance and rehabilitation. <br /> <br />Since the rehabilitation in 2014, the tank has been protected by <br />an ongoing SEH D|B water tank maintenance contract.
