Lake Minnetonka Conservation District <br /> 402 East Lake Street <br /> Wayzata, Minnesota 55391 <br /> No. 4-87 $ 125.00 <br /> DEICING PERMIT <br /> • <br /> WHEREAS, Fairview Hospital <br /> for facility at 4195 North Shore Drive, City of Orono <br /> has paid the sum of One Hundred Twenty-five and no/100' ($125.00) DOLLARS <br /> to the Treasurer of the Lake Minnetonka Conservation District as required by the <br /> Ordinances of said District and has complied with all the requirements of said <br /> Ordinances necessary for obtaining this Permit; <br /> NOW, THEREFORE, By order of the Board of Directors of said District and by virtue <br /> hereof, the said Fairview Hospital <br /> is hereby permitted and authorized to operate deicing equipment in agreement with the <br /> application on file with the District, subject to the following requirements: <br /> that the entire area of open water or weakened ice is enclosed by an opaque <br /> fence which is at least four feet in height; <br /> that the fence and the entire area enclosed by the fence is fully lighted <br /> from one-half hour before sunset to one-half hour after sunrise; <br /> that the fenced area is marked by a minimum of six (6) reflectorized warning <br /> signs at least every fifty (50) feet around the perimeter of the fence; <br /> that an easily identifiable and accessible on/off switch is located near <br /> the unit on the dock; <br /> that the installation is monitored on a daily basis; <br /> in no case may deicing extend or affect areas beyond the dock use area for <br /> which this permit is issued; <br /> lights to be shielded from neighbors and to be high-pressure sodium or other <br /> acceptable yellow bulb <br /> for the period of the 1987-1988 season and <br /> ending April 20, 19 88 subject to all the conditions and provisions of said <br /> Ordinances. <br /> Approved by the Board of Directors of the LAKE MINNETONKA CONSERVATION <br /> DISTRICT this 28th day of October , 19 87 . <br /> Executive Director rank Mixa <br /> Inspected 12-28-87 <br /> Issued 3-2-88 <br /> 12/85 <br />
