Part 2:Replacement Plan Supplement <br /> ❑ Have you completed all of Part 2 (pages 5-8)? <br /> ❑ Did you (or your agent) sign Box 27a or Box 27b on page 8? <br /> ❑ Have you included the necessary attachments for Part 2? <br /> Attachments must include: <br /> ❑ If the project includes any wetland banking(complete or partial), include Application for Withdrawal <br /> of Wetland Credits Form (Section 22) <br /> ❑ If the project includes any project-specific replacements(complete or partial),include: <br /> • Description of Replacement Wetland(s)Construction(Section 23) <br /> • Copy of vegetation management plan(Section 23) <br /> • Scale drawing of overhead view of replacement wetland(Section 26) <br /> • Scale drawing of profile view of replacement wetland(Section 26) <br /> Attachments may also include: <br /> ❑Additional Description of Wetland Impacts charts (Section 19) (if additional space was needed) <br /> ❑Copy of any archaeological or cultural resource determinations or surveys(Section 21) (if you answered YES) <br /> ❑Additional Description of Replacement Wetlands charts(Section 25) (if additional space was needed) <br /> ❑Additional soils information for created replacement wetland(s)(Section 26) (if available) <br /> Note:To deposit surplus wetland credits in the State Wetland Bank,submit a Wetland Banking Application directly <br /> to your LGU (Section 24). <br /> Preparing Your Application for Mailing <br /> El To apply for both state and Federal authorization, your application must include <br /> Part 1 (pages 1-3), the Federal application (page 4), and attachments as indicated <br /> on Final Checklist for Part 1 (Instructions, page 2). <br /> ❑ Your application may also include Part 2 (pages 5-8) and additional attachments <br /> as indicated on Final Checklist for Part 2 (above). <br /> ❑ Make three copies of the entire application and all attachments.
