Builder Acknowledgement Form <br /> Permit #2016-00319 / 3465 Watertown Road ad <br /> Builder Representative Name: C 0 rd oleQYI'J�- <br /> Permit Conditions: Initials <br /> "NOTE CHANGE" Before scheduling an exterior insulation and/or drain tile inspection, a <br /> foundation as-built survey must be submitted and approved by the City or a Stop Work order <br /> will be issued. <br /> Schedule a minimum of one hour for the framing inspection. <br /> Erosion control mechanisms must be installed and inspected by the City prior to any land <br /> disturbing activities. The contractor must provide a minimum of a 24 hour notice prior to <br /> inspection. <br /> Erosion control shall be installed and maintained throughout the entire project and must <br /> remain until vegetation has been established. <br /> No underground sewer within 20 feet of well. <br /> Prior to the issuance of a Certificate of Occupancy an as-built survey and hardcover calculations <br /> must be submitted and approved. <br /> In the event of winter or other extended unfavorable weather conditions(which prevent the <br /> completion of the exterior improvements and/or as-built survey) a Temporary Certificate of <br /> Occupancy(TCO) may be necessary. A TCO requires a $10,000 escrow. <br /> Advisory Comments <br /> Any changes to the exterior/landscaping improvements, i.e. patios,grading, sidewalks, retaining <br /> walls, etc. not currently shown on the approved survey and landscaping plan will require a <br /> separate Zoning Permit application to be submitted and approved prior to the work <br /> commencing. <br /> NOTE: The retaining walls depicted are listed at exactly 4 feet in height. Walls greater than 4 <br /> feet in height must be designed by a licensed professional and plans must be submitted to the <br /> City for review prior to the construction.
