ORONO COP <br /> Building Permit 2012-00589 <br /> SUPPLEMENTAL CONDITIONS <br /> 1. Council approval of the site grading is based on Resolution 6117. Any amendments to the plan <br /> which are not in conformity with City codes will require further Planning Commission and City <br /> Council review. <br /> 2. All fill being used on the site shall meet the City's definition of clean fill. The City Engineer shall be <br /> allowed to inspect any fill being brought to the site prior to it being placed. <br /> 3. The applicants shall work with the City Engineer and/or Willie Gibbs(Septic Manager)to locate and <br /> install protection around the potential septic areas to insure they are not impacted and compacted <br /> during the fill activities. The protection fencing must be inspected and approved by the City prior to <br /> any land disturbing activities. <br /> 4. The applicants shall confirm with the City Engineer acceptable hours of operation, and provide <br /> documentation on the route being used by trucks to bring fill to the site (if applicable). <br /> 5. If fill is being brought to the site,the City Engineer shall inspect the roadways prior to and following <br /> completion of the work to determine whether unnecessary impacts have occurred. The applicant <br /> shall be responsible for the repair to the roadways for any adverse impacts. <br /> 6. On July 13, 2012,the City Engineer reviewed the condition of the driveway and CMP culvert. <br /> a. The CMP culvert under the driveway is in poor conditions,with holes rusted through the <br /> top of the pipe as documented by the attached photos. The culvert should be replaced <br /> as part of this project, matching existing size and elevations. <br /> b. The condition of the driveway and Turnham Road has been documented with digital <br /> photos. A few minor ruts and riffles have been noted on the shared gravel driveway. <br /> Minor expansion cracking and a few areas of alligator crackling have been noted on <br /> Turnham Road.The applicant should be required to review existing conditions and <br /> confirm the City's documentation through an initial condition walkthrough. After <br /> construction is complete,the City Engineer shall review the condition of the driveway <br /> and Turnham Road, and any damage shall be addressed by the applicant. <br /> c. The applicant/contractor shall notify the City 48 hours prior to required inspections. <br /> 7. Construction shall follow the survey and plans as submitted or as required to be updated by the City <br /> Engineer. <br /> 8. The existing home on the property shall be removed prior to issuance of a certificate of occupancy <br /> for the new home. <br /> Owner/Applicant Signature <br />
