The <br /> application does not contain all required information and is incomplete at this time. As specified in District <br /> Rules, review of your application will commence once it is complete. Delay in submitting requested <br /> information may delay the scheduling of this application to a meeting agenda and/or the issuance of your <br /> permit. The following information or materials were not present in your application: <br /> $10 Application fee, in the form of a check made out to MCWD (cash will not be accepted) <br /> Both pages of the permit application completed and signed by the applicant(property owner) <br /> and representative. NOTE: The application may be signed by a duly authorized agent if a letter bearing <br /> the original signature of the property owner is submitted stating the agent is is the authorized by the <br /> landowner for all matters relating to the permit. <br /> Locations of proposed runoff control, erosion prevention, sediment control and temporary and <br /> permanent soil stabilization measures, including: <br /> o Perimeter control along all roads and trails. <br /> o Perimeter control at the bottom of all slopes leading off site or toward water resources. <br /> o Perimeter control and/or cover around/on all large stockpiles. <br /> o Crushed rock or existing paved construction entrance, only 1 allowed per site. <br /> A permanent stabilization plan that states the following(can be written on site plan): <br /> ❑ Addition of at least 6" of topsoil to all disturbed areas. <br /> ❑ Method for establishing permanent vegetative cover. <br /> The MCWD would like to remind the applicant that construction is not to begin on site until a permit has been <br /> issued and received. Submittal of application materials does not guarantee permit issuance.
