The employee must notify employee's department head or designee of the need for sick <br />leave within one-half (1/2) hour of the beginning of the work day. Lack of proper notification <br />may be cause for disciplinary action, and claiming sick leave when physically fit may be cause <br />for disciplinary action, including transfer, suspension, demotion, or dismissal. <br />In order to be eligible for sick leave with pay, an employee must: <br />a. Report the reason for the absence within one-half (1/2) hour of the beginning of <br />the work day to the department head. <br />b. Submit a medical certificate for any absence of any duration, if required by the <br />department head and City Administrator. <br />The employee must complete a Request for Leave form before the employee will be <br />granted sick leave pay. The use of sick leave will be prorated in units of not less than one (1) <br />hour. An employee whose illness extends beyond the sick leave credit available may use <br />available vacation leave and/or apply for a medical leave of absence. <br />Employees using earned sick leave shall be considered to be working for the purpose of <br />accumulating vacation or sick leave. <br />An employee who has been unable to work for a period of time because of illness or <br />accident may be required before being permitted to return to work to provide medical evidence <br />that employee is again able to perform all significant duties of the position in a competent <br />manner without hazard to employee or others. <br />An employee who meets the other requirements of this section who is receiving worker's <br />compensation payments may be granted sick leave pay for the amount of the difference between <br />employee's worker's compensation payment and employee's salary to the extent that employee's <br />sick leave is accrued. <br />An employee may use sick leave for absences due to illness of the employee's immediate <br />family for such reasonable periods as the employee's attendance with the immediately family <br />member may be necessary, on the same terms the employee is able to use sick leave benefits for <br />the employee's own illness. For the purpose of such sick child care leave, a "child" means an <br />individual under 18 years of age or an individual under age 20 who is still attending secondary <br />school. Sick leave may be used to care for a healthy child after birth, adoption, or foster care. <br />Employees covered by a collective bargaining agreement shall be governed by said <br />collective bargaining agreement in regard to policy on sick leave. <br />34 <br />142286x12 <br />
