Observation Report Gage aQ 460 Tonkawa Road <br /> 010 <br /> Pie's scope of work was to perform the following regarding the Gage residence located at 460 <br /> Tonkawa Road Long Lake,Minnesota: <br /> A. Travel to and from site in Long Lake,Minnesota. <br /> B. Meet for inspection on Monday,September 26 at 2pm. <br /> C. Observe structure for reported damage from lightning strike. <br /> D. Provide verbal and written report of findings with recommendations for repair. <br /> The scope of our observation included the following: <br /> September 23,2016 Discussion with the adjuster,Mr.Tony Brian D.Erickson,P.E., <br /> Skajewski,Chubb,to review the assignment RRC <br /> and determine the scope of work at the start <br /> of the project. <br /> September 26,2016 Public records search of the property as Shannon W.Pierce,P.E. <br /> available on the Hennepin County website. <br /> September 26,2016 Observations,measurements,and <br /> photographic documentation performed. <br /> r <br /> A. Pie conducted the site evaluation on September 26, 2016. Mr. Tony Skajewski, from <br /> Chubb, Mr. Steven Steinborn, caretaker representing the homeowner, and Mr. Greg <br /> Newtson,from Giertsen,were present during the site visit. <br /> B. Lightning struck the roof ridge at the north end of the house.The lightning strike traveled <br /> through the north window wall and terminated at a wired smoke detector mounted to a <br /> vertical window mullion on the north wall. <br /> 1. The room at the north end of the house is considered to be a living/sitting room. <br /> a. The room has a vaulted ceiling/roof,and is approximately 16-feet tall from <br /> the floor to the roof ridge beam. <br /> 1. The timber roof rafters are approximately 6-inch by 8-inch <br /> (nominal)members spaced at approximately 4-feet,6-inches apart. <br /> 2. The vaulted timber roof rafters are supported by presumably <br /> wood-framed walls(low)and a timber ridge beam(high). <br /> 3.
