� � . <br /> Sec.13.04 RIGHTS OF SLJ&TECTS OF DATA <br /> Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forrh in this secdon. <br /> Subd.2. Information req�red to be given individual. An individual asked m supply private or confidendal dara conceming himself <br /> shall be infoRned of: (a)the purpose and inte�ed use of the requested data within the collecting state agency,poliucal subdivision,or statewide <br /> rysum;(b)whecher he may refuse or is legally required w supply the requesud data;(c)any lrnown consequence arising from his supplying or <br /> refusing to supply private or confidential data;and(d)the idendry of other persons or endaes au[horized by state or federal law to receive the dara. <br /> This requirement shall not apply when an individual is asked to supply investigadve data, pursuant to secdon 13.82, subdivision 5, to a law <br /> enforcement officer. <br /> The commissioner of revenve mav ulace the noece reouired under thi �bdivision in ihe individual income taz or orooertv tax refund <br /> instruceons insread of on those fortns. <br /> Subd.3. Access to data by individual. Upon request to a responsible authoriry,an individuai shall be informed whether he is the <br /> subject of stored data on individuals,and whether it is class�ed as public,private or confidenaal. Upon his further request,an individual who <br /> is the subject of stored private or public data on individuals shall•be shown the dara without any charge to him and,if he desires,shall be informed <br /> of the contenc and meaning of that dara. After an individual has been shown the private dara and informed of iu meaning,the data need not be <br /> disctosed to him for six months thereafter unless a dispute or ac6on pursuant to this secaon is pending or addirional data on the individual has been <br /> collected or creared. The responsible authoriry shall provide copies of the private or public data upon request by the individual subjecc of the data. <br /> The responsible aurhority may require the requesang person to pay the actual cosu of making,certifying,and compiling[he copies. <br /> 'Iha responsible authoriry shall comply immediately,if possible,with any request made pursuant to this subdivision,or wi�hin five days <br /> of the dau of the request,exctuding Samrdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with <br /> the request wi[hin that time,he shall so inform the individual,and may have an additional five days within which to comply with the request, <br /> excluding Samrdays,Sundays and legal hoHdays. <br /> Subd.4. Procedure whea data is not accurate or complete. An individual may contest the accuracy or completeness of public or <br /> private daa concerning himself. To exe�sise this right,an individual shall notify in wridng the responsibk authoriry describing the fladire of die <br /> disagreemenc The responsible authoriry shap within 30 days either: (a)correct the dara found to be inaccurate or incomplete and aaempt to noafy <br /> pact recipienrs of inaccurate or i�omplete data,including recipienrs named by the individual;or(b)noafy the individual that he believes the data <br /> to be correct. Data in d'upute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. <br /> The dererminaaon of the responsible authoriry may be appealed pursuant to the provisions of the adminis�aave procedure act relating <br /> to contested cases. <br /> UATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd. 2, "Rights of subjects of data", we would like to inform you that your <br /> request for a permit or license from the �iry of Orono or any of its departments may require you to furnish certain <br /> private or confidential information. <br /> You are notified that: <br /> 1.
