<br /> 2.Contad the Hennepin County Pertnit Office at 612-596-0339 a minimum of 2 business days prior to starti�g construction.
<br /> 3.After drivevray constnx�ion is completed,peraons requesting the installation must notify the Pertnits Office that such work has been completed and is ready forflnal
<br /> inspection and acceptance by Hennepin County.
<br /> 4.No changes or alterations in entrances mey be made at arry time withoul written pertnission from the County Ergineer or designee.
<br /> 5.Approval of this permit does not tleny the County Engineer the future right to revise,relocate or dose any entrance to expedite the movements oI vehicular ar pedestrian
<br /> Veffic.The County retains the right to Gose said entrance(s)at its discretion,based upon the necessity for Vaffic regulation,conVol and safery.
<br /> 6.Existing tlriveways are not automatically perpetuated.In the event of a change in land use or a major change in the traRc paltem of the exis�ting tedlily,new driveway
<br /> applications may be required by the County.
<br /> 7.Indement weather shall be reason far delay of tlie projed start or suspension oT the project as detertnined by Hennepin County.
<br /> 8.Pertnits or copies shall be kept on the work site,while worlc is in progress,in the custody of the individual in charge,and shall be exhibited upon request by any county
<br /> offiaal.
<br /> 9.These generel pertnit requirements and spedfications ahall be considered as fortning an integral part of each and every pertnit issued for operations on couMy roadways.
<br /> The work authorized by this pertnit shell be done et such time and in such manner as shall be consistent with the safety of the public and shall confortn to all requirements and
<br /> standards of the County of Hennepin as herein spedfied.If at any time,it shall be found by Hennepin County that the work is not being,or has not been,propeAy perfortned,
<br /> the Pertnitlee,upon being not�ad,shall immediately teke the necessary steps,at their own enpense,to place the work in confortnance to seid requiremeMs or standards.In
<br /> addition,no residential driveway may exceed 22 feet in widTh and no commertlal driveway shall exceed the width dictated by the Hennepin County Access Management
<br /> Guidelines.
<br /> 10.In the event of failure or neglect,will or not,by said Permittee or Pertnittee's employees,to perfortn and comply with the prescribed conditions,resUidions and regulations,
<br /> the Transportetion Departrnent may revoke and annul this pertnit and order said Pertni$ee to immediately remove any and all sWdures or property belonging to said
<br /> Pertnittee from the legal limits of the roadway or county propeRy.
<br /> 11.The installation shall be mede in confortnity with ell applicable laws,reguladons and codes covering said installations.AII installations shall be made incoMortnity with
<br /> regulaations of govemmental agendes for the protectlon of the public.
<br /> 12.Americans with Disabilities Act-Any work in the County's rigMs-of-way that impads any pedestrian fadlitles requires that the restoration of the pedestrian fatllities
<br /> impaded be restored to confortn to current ADA requirements and standards.
<br /> 13.If pavement,roadway or rigM-of way is damaged;same shall be restored,in a timely manner to its original or e better condibon,at the sale expense of the Pertnittee.
<br /> Pertnittee's tailure to corred such damage in a tim�y manner will result in Hennepin County,fhrough its ovm forces or contradors,making said repairs and billing Pertnittee
<br /> for all costs and expenses relatetl thereto.Pertnittee shall make payment to Hennepin County within fifteen(15)days of receipt of seid bill.Pertnittee agrees that if it faiis to
<br /> make payment of said amaunts wiThin fhe specific time,to Hennepin Counry,it shall pay all costs relaUng to debt calledion,induding,but not limited to,couR costs,frees,
<br /> interest and attomeys fees.
<br /> 14.No driving onto highway from ditch or driving on shoulders or over curbs where damage wili occur.
<br /> 15.It shall be further understood that the Permittee will be responsible for maintenance or repeir of any and all failurea due to settlement,erosion,ladc of vegetation growth,
<br /> rutting or oiherjob related problems for a period of three(3)years after projed complebon and Notice of Work Completion form has been received by Hennepin County.
<br /> Pertnittee shell perfortn all said mairdenance.Immediately after it has been notified of said defidency.Pertnittee shall be responsible for all costs and expenses related to said
<br /> maintenance.Pertnittee's failure to perfortn said maintenance immediately will resuit in Hennepin Count,through its own forces or its contractors,perfortning said maintenance
<br /> and billing Pertnittee for all costs and expenses related thereto.Permittee shall make payment to Hennepin Counry within fifteen(15)days of receipt of said bili.Pertnittee
