"Type IV - performance" <br /> Monitoring and Maintenance Disclosure <br /> Property address: <br /> The septic system designed for this site has been classifed as a Type IV system and will therefore require <br /> ongoing monitoring and maintenance for the life of the system. <br /> It is the owners responsibility to discuss these requirements (as listed on the permit) and their <br /> respective intervals and costs with their septic designer and/or maintenance provider. <br /> Monitoring and maintenance may include (but is not limited to): <br /> -Manufacturer required maintenance <br /> -Tank pumping <br /> -Taking Lab samples <br /> It is the owners responsibility to hire an M.P.C.A licensed septic professional to perform this work, and <br /> for that professional to submit an annual report to the Local Unit of Governement. <br /> All other tong term septic system costs such as repairs and emergency service calls outside of any warranty <br /> coverage (if applicable), are outside the scope of this disclosure. <br /> I acknowledge and understand these requirements, and agree to contract a licensed professional to <br /> monitor and maintain this system. <br /> Owner Signature Date <br />
