The righrs of individual on whom[he dara is stored or to be scored shall be as set farth in this secdon. <br /> Subd.2. Informaaon reqirired to be givea individual. An individual asked to supply private or confidendal dara concemiag himself shall <br /> be informed of: (a)the purpose and inceaded use of the requesud dara wichin the collecting�tare agency,polirical subdivision,or statewide rysum: <br /> (b)whether he may refuse ot is legally required to supply[he requested data:(c)any Irnown coasequence arising from his supplying or refusing to supply <br /> • privace or confidenaal data;and(d)the idendry of other persoas or enrides au�horized by state or federdl taw to receive rhe data. Access to data by individual. Upon requesc to a responsible authoriry,an individual shall be informed whether he is the subject <br /> of stored dara on individuals,and whecher ic is classified as pubtic,private or confidendal. Upon his fur[her request,an individual who is the subjecc <br /> of stored private or public dara on individuals shall bo shown�he dara wichout any charge to him and;if he desires,shall be informed of the content <br /> and meaning of[hat dara. Afcer an individual has been shown the priva[t data and informed of its meaning,the dara need not be disclosed to him for <br /> six monrhs therea[cer unless a dispute or acaon pursuan�to chis secaon is pznding or addiaoaal data on[he individuai has been collecced or crea[ed. <br /> The respansibie authoriry shall provide copies of the privace or public data upon request by the individual subject of�he dara. The responsible authoriry <br /> may require che requesting person to pay[he accual coscc of making,certifying,and compiling the copies. <br /> The responsible authoriry shall comply immediacely,if possible, with any request made pursuant to this subdivision,or wichin five days of <br /> the dace of the request,excluding Sacurdays,Sundays and legal holidays,if immediace compliance is not possible. If he cannoc comply wi[h the requesc <br /> wichin chac ame,he shall so inform the individual,and may have an addidonal Five days within wluch to comply wi[h the request,excluding Saturdays, <br /> Sundays and legal holidays. <br /> Subd.4. Pzocedure when data is not accurate or complete. An individual may contest the accurdcy or completeness of public or private <br /> data concerning himselF. To exercise chis ri¢hc,an individual shall nodfy in wridng the responsible authoriry describing rhe nature of the disagreemeat. <br /> The cesponsible auchoriry shall within 30 days eicher: (a)correct the data found to be inaccurate ot incomptete and aaempt to nodfy past recipienrs of <br /> inaccurate or incomplete data, including recipienrs named by the individual; or(b)nocify the individual that he believes the data to be coaecG Data <br /> in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. <br /> The decerminadon of the responsible authoriry may be appealad pursuant to the provisions of the adminisnarive procedure act relaang to <br /> contesced cases. <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd.2, "Rights of subjects of data",we would like to inform you tha[your request <br /> for a permit or license from the City of Orono or any of its departments may require you to furnish certain private or <br /> confidential information. <br /> You are notified that: � <br /> i.
