Monthlv Em�lovment Utilization Reports (Form CC25n - Must be submitted by the <br /> 10th calendar day of each month (see Appendix C). Reports submitted for construction <br /> contracts must cover the employment hours worked by the Company and its <br /> ' subcontractors at the project site. Reports submitted for service matntenance contracts <br /> must indicate the employment hours worked at all of the sit�. <br /> ' 2. E�nalovment Sumrrr�rv Reaort for Constru�tton Projeats (Fo�n CC�;gO) - Must be <br /> attached to the Monthly Employment U�lizadon Reports 'for 'the report pertod and <br /> submitted by the 10th calendar day of each month (see Appendix D). <br /> ' We will also evaluate our u�lization of minority and women �mpioyees on a monthly basls. if <br /> our u�liza�on of such employees falis below.the goals established in our approved Affirmative <br /> ' Adion Pian, we will submit to the Hennepin County Targeted Contract Services Division, along <br /> with the monthly reports listed above, a written Narrative Report that indudes the foliowing <br /> information: <br /> � (a) An explanation of the•reasons why the utilization has failen below the goals; and <br /> (b) The affirtnative action steps that our subc�ntractors and we took to achieve our <br /> ' goals during the repofing period. <br /> In addition, we will provide assistance to the Hennepin County Targeted Contract Services <br /> , . Division during on-site visits and desk audits. <br /> The Company will notify subcontractors of their responsibilfti� for Equat: Employment <br /> ' Opportunity and Affirmative Action under this Affirmative Action Plan and we will hold them <br /> accountabie and impose sanctions where appropriate. <br /> ' All Company empioyees shall be told.of their rights and a notice of their rights shall be posted. <br /> The employees shall be told of their right to flle a complaint with an enforcement agency and <br /> where to file such a complaint.
