Attached is your original license. According to City Code Section <br /> 26-36, this license must be displayed in a public location within the salon. Referenced code <br /> sections are enclosed for your information. <br /> As noted on the license, the following massage therapists are covered under the Seidou Salon <br /> Spa massage therapy license: <br /> • Meryl Verkins <br /> Any individual who provides massage therapy services must be covered under the current <br /> license. Please be aware, performing massage therapy services without a license is a violation <br /> of City Code Section 31-11. If you wish to add new massage therapists to your license prior to <br /> the annual renewal date1 the individual must complete the attached Application for Massage <br /> Therapy License, Part III and return it to the City of Orono.
