� Sec.13.04 RIGHTS OF S[JBJECTS OF D�T� Subd. 1. Type of data. The righcs of individual on whom che data is stored or ro be storcd shall be as sec for[h in this secaon. Subd.2. This requiremeoc shall not apply when an individual is asked to supply invesdgaave dan, pursuaa�to secdon 13.82, subdivision 5, co a law enforcemenc ofFicer. Access to data by individual. Upon requcst to a responsible auchoriry,an individual shall be informed whe�her he is the subjecc of scored dara on individuals,and whe[her it is classified as public, private or confidendal. Upon his furthzr request, an individual who is the subject of stored private or public dam on ir.dividuals shall be shown che dara wichout any charge to him and,�if he desires, shall be informed of�he contenc and meaning of chat dam. Aher an individual has been shown che privace data and informed of ics meaning,the dara need not be disclosed to him for six monchs[hereafter uriless a dispuce or acdon pursuanc co chis secdon is ptnding or addidooal data on the individual has been collecc:d or creat�d. The responsibie auehoriry shall provid�copies of[he private or public data upon request by the individual subjecc of the data. The responsible authoriry may require the requesdng person to pay che actual costs of makin¢, certifying,and compiling the copies. The responsible au�horiry shall compty immediately, if possible, wich any request made pursuant co this subdivision,or wichin five days of the dace of�he reques�,ezcluding Sanirdays,Sundays and legal holidays,if immediace compliance is not possible. If he cannot comply with the requesc wi[hin fiat rime,he shall so inform chc individual,and may have an addidona!five days wi[hin which to comply wirh the request,exciuding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or pri�•ace data concerning himsetf. To exercise�his right,an individual shal!noafy in wricing che responsibie auchoriry dzscribing the nacure of the disagreemene. The responsible authoriry shall within 30 days either: (a)correct the data found to be inaccurace or incomplece and aaemp�to noafy past recipien�s of inaccurate or incompte�e data, inciuding recipiena named by che individual; or(b)notify[he individual thac ha believes the data to be correcc. Data in dispuce shall be disclosed only if the individual's statemenc of disagrecment is included with the disclosed data. The decerminaaon of the responsibtz authoriry may be appealed pursuant to the provisions of[hz admuusaarive procedure act relating to contes:ed cases. You are notified that: 1. The information you furnish will be used to determine your qualification for the permit or license requested. 2. You may refuse to supply data, buc refusal may require that the City deny the permit or licease. 3.
