Box 10853 White Bear Lake, MN 55110 Brian Humpal <br /> 651-492-7550/Brian@Midwestsoiltesting.com MPCA Licensed Advanced Inspector <br /> SUBSURFACE SEWAGE TREATMENT SYSTEM (SSTS) COMPLIANCE REPORT <br /> Date: May 16, 2018 Time: 9:15 AM Owner: Paul Pesek <br /> Inspection Address: 1235 Lyman Avenue, Orono, MN 55356 <br /> REPORT SUMMARY <br /> I have performed an "MPCA Compliance Inspection" on this system and have reviewed <br /> the limited records on file with the City of Orono and a previous compliance inspection from <br /> April 25,2018 on file with the owner. This very old system (installed in 1984) consists of two <br /> pre-cast septic tanks,a pre-cast lift tank,and a mound. It should be noted that the average life <br /> expectancy of a septic system is approximately 30 years. <br /> According to city records, the mound surfaced in 1987 and at the time additional fill was <br /> brought in for the base of the mound. No reports of surfacing have been reported since. <br /> Although not a compliance criteria, the lift pump alarm may not be working properly. <br /> The alarm should be verified that it works and if found not working it should be replaced as soon <br /> as possible to notify the occupants of the house in the event of a pump malfunction. In addition, <br /> there are two large trees adjacent to the mound red bed and a cluster of smaller trees on the rock <br /> bed that should be cut flush to the ground, taking special care that no equipment be driven on the <br /> mound. <br /> Predicated on my inspection of the system and my review of the records records,it is my <br /> opinion that this system presently meets MPCA minimum compliance inspection requirements. <br /> Inspect Minnesota and Midwest Soil Testing have been hired to perform a compliance <br /> inspection of this SSTS for compliance with local ordinances pursuant to Minn. Stat. § 115.55 <br /> (2013). This compliance inspection covers only the criteria required by Minn. Stat. § 115.55 <br /> Subd. 5a (2013) and Minn. R. 7080.1500 (2011). A compliance inspection is an indication of the <br /> current compliance status of the system and does not guarantee the performance or longevity of <br /> this system beyond the date of inspection, as it is impossible to determine the future performance <br /> of any system. Inspect Minnesota and Midwest Soil Testing disclaim any use of this compliance <br /> inspection beyond determining SSTS compliance pursuant to Minn. Stat. § 115.55 Subd. 5a <br /> (2013) and Minn.
