Each Permittee must create a map of their facility. The map shall be of a 1:24,000 or larger scale. What this means is that
every inch on the map represents 2,000 feet on the ground. The map needs to contain:
a) The surface water body's name which receives your facility's discharges(if the receiving water is beyond the scale of
the map,an arrow in the direction of the water and its name is sufficient).
b) Identify all impervious surfaces at the facility.
c) Direction of stormwater flow(indicated by arrows).
d) Identify location of all industrial activities and significant materials.
e) Location of structural Best Management Practices(BMPs).
fl Location of all impaired waters, within a mile of any monitoring location (include name and impairment).
g) Name and location of any water described in Appendix A,within a mile of any monitoring location.
h) Location of all storm sewer inlets.
i) Location of all loading dock drains(be sure to include those connecting to storm sewer).
j) Location of every Benchmark Monitoring Location(give a unique ID starting with BML01, BML02, etc.,and be sure to
denote if these are located within a mile of impaired or appendix A listed waters).
k) Location of every Effluent Monitoring Location (give a unique ID starting with EML01, EML02, etc.).
I) Location of any non-stormwater discharges.
4. Assessment of Industriat Activities and Significant Materials
It is important for the Permittee to know what materials and activities are taking place and potential sources of pollution to
stormwater. Understanding these activities and materials will enable you to more effectively manage stormwater. The Permittee
should create a list all applicable industrial activities taking place at the facility as referenced in Appendix D, and also include
other activities taking place(not limited to):
• Fueling
• Boat oil changes and light maintenance
• Liquid storage tanks 1 - 2000 underground fuel tank
B. Assessment of materials and associated pollutants
When considering all of the significant materials at your facility evaluate whether any material is stored or handled that
may potentially be a source of pollutants to stormwater. Include a list all of the materials and the pollutants associated
with the identified significant materials.A Material Safety Data Sheet(MSDS) may be useful for this purpose.
Material Pol lutant
Ex. sawmill blades, lubricants Zinc%il
Gasoline Gasoine
When considering materials to include:raw materials, intenediate products,by-products,final products, and waster products.
5. Storm Water Control Measures Implementation (Part III implementation)
The SWPPP must document all of the stormwater control measures required in Part III of the Permit, including BMPs that are
used to manage stormwater. The Permittee should include all BMPs within each stormwater control category specified in Part
II I of the permit. The location and type of BMP along with implementation description of how these BMPs will work and be
measured should be included in the SWPPP.
Good housekee in
BMP Im lementation date Area/Activi Potential ollutants
Rock sediment tra Cleaned out 5-14 Browns Ba side N/A
Natural Grasses Planted 5-14 Browns Ba Side N/A
www.pca.state.mn.us • 651-296-6300 • 800-657-3864 • TTY 651-282-5332 or 800-657-3864 • Available in altemative formats
wq-strm3-28 • 11/7/11 Page 2 of 7
