Sec.13.04 RIGIiTS OF S[TBJECTS OF DAT?, <br /> Subd. !. Type of data. 'ihe righcc of individual on whom che data is stored or to be stored shall be as set forrh in this secaon. <br /> Subd.2. Iaformation reqtrired to be given individual. A,c►individual asked to supply private or confidendal data conceming himself shail <br /> be informed of: (a)[he purpose and incended use of the requesred data within the cotlecang�tate agency,polidcal subdivision,or statewide rysum; <br /> (b)whether he may refuse oY is legally required to supply ehe requesud dara:(c)any known consequence arisiag from his supplying or refusing to supply <br /> private or confidendal data;and(d)che idendry of ocher persons or enriaes authorized by scate or federal law to nceive the data. 'Ihis requiremen�shall <br /> not apply whzn an individual is asked to supply invesdgadve dara,pursuanc to secdon 13.82,subdivision 5,to a law enforcement officer. <br /> The commissioner of rc��enue mav olace the nodce reauired under chis subdivision in the individual income tax or propertv tax refund <br /> instructions inscead of on�hose forms. <br /> Subd.3. Access to data bv indi�iduai. Upon request ro a responsible aurhoriry,an individual shall be informed whether he is the subject <br /> of stored data on individuals,and whe[6er it is classified as public,private or confidencal. Upon his further request,an individual who is the subjecc <br /> of stored private or public data on individuats shall be shown the dara wichout any charge to him and:if he desires,shall be informed of[he content <br /> and meaning of chat data. After an indi iidua!has bten shown the priv�m data and infortned of its meaning,..the data need not be disclosed to him for <br /> six mondu thereaiter unless a dispute or acdon pursuanc to this secrion is pending or addiaonal data on the individual has been collected or creaced. <br /> The responsible authoriry shall provide copies of the private or public data upon mquesc by the individual subjcct of the data. The responsible authoriry <br /> may nquire the requesdng person to pay the actual cosu of making,certifying,and compiling the copies. <br /> The responsible auchoriry shall comply immediately,if possible,with any request made pursuant to this subdivision,or wi[hin fivt days of <br /> the date of the requesc,exeiuding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with che requesc <br /> wichin thac rime,he shall so infor�[he individual,and may have an addidooal five days wi[hin which to comply with the request,excluding Saturdays, <br /> Sundays and legal ho[idays. � ' � <br /> '� <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest[he accuracy or completeness of public or pri�•ate <br /> data conceming himself. To exercise chis righ�an individual shall nodfy in wriring the respon�ible authoriry describing rhe nacure of the disagreemenc. <br /> The nsponsible authoriry shall within 30 days either: (a)correct the data faund m be inaccurate or incomplete and aaempt to nodfy past recipienu of <br /> inaccurate or incomplem data, including recipients named by the individual;or(b)norify the indi�idual tha[he believes the daca to be correcG Data <br /> in dispu[e shall be disclosed only if the individual's statemenc of disagreement is included wi[h the disclosed data. <br /> The decerminaaon of the rzsponsible au�horiry may be appealed pursuant to the provisions of ehe adminisaarive procedure act reladng to <br /> contested cases. , <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04,Subd.2, "Rights of subjects of data ,we would like to inform you that your request <br /> for a permit or license from the City of Orono or any of i[s departments may require you to fumish certain priva[e or <br /> confidential information. <br /> You are notified that: <br /> 1. The information you furnish will be used to determine your qualification for the permit or license requested. <br /> 2. You may refuse to supply data, but refusal may require that the Ciry deny the permit or license. <br /> 3, The information may be shared with ocher local, state or federal agencies to the extent necessary to process <br /> the permit or license. <br /> 4.
