The righcs of iadividual on whom rhe data is stored or co be scored shall be as set for�h in this section. <br /> Subd.2. After an individual h�s been shown che private dara and informed of its meaning,the dara need not be disclosed to him for <br /> siz monrhs thereafter unless a dispute or acaon pursuant to this secaon is pending or addidonal data on the individual has been collected or created. <br /> The responsibie au[horiry shall provide copies of cha private or public data upon request by the individual subject of ehe dara. The responsible auchoriry <br /> may require che requesring person to pay the acaial costs of making,certifying,and compiling the copies. <br /> The responsible aurhoriry shali comply immediately,if possible,wi[h any requesc made pursuant to this subdivision,or within five days of <br /> the dace of the requesc,excluding Saturdays,Sundays and legal holidays,if immediace compliance is not possible. If he cannot comply with the requesc <br /> within chac time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays. <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private <br /> data concerning himself. To exercise chis right,an individual shall nodfy in writing the respoasible authoriry describing the namre of the disagreement. <br /> The responsib(e authority shall within 30 days eick:er: (a)correct the data found to be inaccurdce or incomplete and atcempt to nodfy past recipients of <br /> inaccurace or incomplete data, including recipiencc named by the individual;or(b)notify the individual that he believes rhe dara co be correcc. Data <br /> in dispu[e shall be disclosed only if the individual's statemenc of disagreement is included wich the d'uciosed dara. <br /> The decerminadon of the responsible au�horiry may be appealed punuanc to the provisions of the adminisaarive procedure act relating to <br /> contested cases. <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd.2, "Rights of subjects of data", we would like to inform you that your request <br /> for a permit or license from the City of Orono or any of its departments may require you to fumish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1. The information you furnish will be used to determine your qualification for the permit or license requested. <br /> 2.
