r <br /> _ ♦ <br /> Sec.13.04 RIGHTS OF SUBJECTS OF DATA <br /> Subd.1. Type of data. The rights of lndivldual on whom the data Ls stored or to be stored shall be as set forth in this section. <br /> Subd.2. Information required to be given iodividual. An indi�idusl uked to supply pri�•ate or contidential data concerning himselishall be <br /> informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdlvision,or statewtde system;(b) <br /> whet6er he may refuse or is legally required to supply the requested data;(c)any known consequeuce arising from his supptying or refusing to supply <br /> private or conRdential data;and(d)the identlty of other persons or eotioa suthorized by state or federal law to recelve the data. Access to data by indlvidual. Upon request to s responsible authority,an indi�idusl shall be lnformed whether he Is the subject of <br /> stored data on individuals,and whether it is classifled as public,privau or rnnfidentfal. Upon his furt6er request,an indicidual who is t6e subject of <br /> stored private or publlc data on indlviduals shall be shown t6e data withoat any charge to him and,if he desires,shall be informed of t6e content and <br /> meaning of t6at data. After an individual 6as been shown the private dita and iniormed of Its meaning,the data need not be dlsclosed to him tor sia <br /> months thereafter unless a dispute or actlon pursuant to this section is peading or additionsl data on the individual has becn rnllected or created. T6e <br /> responsible suthority shall provide copies otthe prlvste or publ(c data upon request by the individual subJeM of the data. T6e responsible authority may <br /> require the requesting person to pay the aMual costs of maldng,certif}iag,sod compiling t6e copies. <br /> The responsible authority shall comply immedlately,If possibfa with any request made pursuant to thls subdivision,or within tive days otthe <br /> date of the request,excluding Saturdays,Sundays and legal6olidays,if immediate rnmpl(ance is not possible.If he cannot comply with the request within <br /> that time,6e s6a11 so inform the Individual,and may hsve an additional fi�-e days within which to compiy with the request,ezduding Saturdays,Sundays <br /> and legal holldays. <br /> Subd.4. Procedure when data is not accurate or rnmplete. ,�n individual may contest the accuracy or completeoess of publlc or prlvate data <br /> concerning himself. To exercise this right,an indlvidual shsll notify ia writing t6e responsible aut6ority describing the nature of the disagreement The <br /> responsible authority shall withln 30 days etther: (a)correct the data found to be inaccurate or inrnmplete and attempt to notify past reciplents of <br /> inaccurate or incomplete data,including reclpients named by t6e individnal;or(b)aotify the indi��dusl that he believes the data to be rnrrect. Data in <br /> dispute shall be dlsclosed only if the indlvidual's statement of disagreemeat ls included with the discfosed data. <br /> The determination of the responsible authority may be appeakd pursuant to the provisions ot the administratice procedure act relating to <br /> rnntested cases. <br /> DATA PRI�:�CY ADVISORY <br /> In accordance with M.S.13.04,Subd.2,"Rights of svbjects of data",we�-ould like to inform�•ou that your request <br /> for a permit or license from the City of Orono or any of iu departments may require you to furnish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1.
