Sec.13.04 RIGHTS OF SUBJECTS OF DATA , - Subd. 1. Type o[data. 'Ihe rights of individual on whom the data is stond or to be stoced shall be as set forth in this secrion. Subd.2. Iaformatlon required to be given individual. An individual asked to supply private or confidential.data conceming himself shall be informed of: (a)the purpose and intended use of the requested data within the coltecting iate agency,political subdivision,or statewide rystem; (b)whether he may refuse or is legally rcquired to supply the rcquested data;(c)any known consequence arising from his suppiying or rcfusing to supply private or confidendal data;a�(d)the identiry of other persons or endties authorized by state or federal law to meive the data. This reqnirement shall not apply when an individual is asked to suppiY investigarive data,?unvant to secrion 13.82,subdivision 5,ro a law eaforeement officer. The commissioner of revenue mav olace the notice reauircd under this subdivision in the individual income tax_or prooertv tax refund insuucdons instead of on those fomu. Subd.3. Accas to data by indiridual. Upon rcquest to a responsible authoriry,an individual shall be informed whe[her he is the subject of storcd data on individuals,and whether it is classified as pubtic,private or confidential. Upon his further request,an individual who is the subject of stored private or public data on individuals shall be shown the daa without any charge to him and,if he desires,shall be infoened of the conant and meaning of that data. After an individual has been shown the private data a�infortned of its meaning,the data need not be disclosed to him for siz months tliereafter unless a dispute or action putsuant ro c'�is section is pending or addirional data on the individual has been wllected or created. The responsible authoriry shall provide copies of the private or publio data upon request by the individual subject of the dam. The responsible authoriry may require the requesting person to pay the actual costs of making,cectifying,and compilin;the copies. - The tesponsible authority shall comply immediately,if possible,arith a�ry request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal hotidays,if imtnediate compliance is not possible. 1f he cannot comply with the request within that time,he shall so infocm the individual.and may have an sdditional five days within which ro comply with the mryest.ezcluding Satucdays. Sundays and legal holidays. � ' Subd.4. Procedure when data is not acc�ate or complete. An individval may wntest the accuncy or completeness of pnblic or private ; da'ta conceming himself. To exercise this right,an i�ividual shall nntify in writing the responsible suthority deacn'bina the namre of the disagreement. � The responsible authoriry s6a11 withia 30 days either. (a)correct the data found m be inaccunte or incomplete aud atoempt to notify past reciPients of inaccuraoe or i�uomplete data,including tecipients named by du iniividual;or(b)notify the individual that he bdieves the data to be correct. Data in dispnu shall be disclosed only if the individual's stacement of disagree�nt is included with the disclosed daa. The determination of the responsible authority may be appealed pursuanc to the provisions of the administrative procedure act celadng to contested cases. You may refuse to supply data, but refusal may require that the City deny the permit or license. 3.
